Preparing for your initial consultation helps ensure your appointment runs smoothly and allows for accurate assessment and planning.

Before Your Appointment

Please arrive 10 minutes prior to your appointment to complete a registration form.

If you have not already emailed or faxed your referral and related reports, please bring the following with you:

  • Referral letter from your GP, family physician, or other doctor

  • Your Medicare card and private health insurance details (if applicable)

  • Details of current medications

  • Any CT, MRI, or PET scans (on USB or CD), including reports

  • Any other relevant investigation results (e.g. blood tests)

Why This Is Important

During your evaluation or treatment, imaging such as CT scans or other investigations may be required to determine the cause of your symptoms or assess your progress.

If relevant information is not available at the time of your appointment, this may delay your diagnosis or treatment. In some cases, repeat investigations may be required to complete your assessment.
